  • 4. Photocopy of current year's Business / Mayor's Permit

  • 5. Photocopy of valid DTI Business Registration

  • 7. Online TIN Verification Screenshot / TIN Verification Slip

  • 8. Proof of Billing / Brgy. Clearance under Borrower's name

  • 9. Primary Valid ID with 3 specimen signature of Borrower & Spouse (if married)
